The questions below are intended to provide the necessary information the nomination committee will need to make their selections.  Unless indicated, you are not required to complete every section.

The selection committee may conduct follow-up interviews to obtain supplemental information on the Nominee.  This interview will not replace an incomplete nomination form.

Please complete the following questions in the space provided. It is recommended that you copy and paste your content from another document. 

To provide the most compelling application, please keep the following in mind:  

Be specific and succinct.  Bullet points are perfectly acceptable.  
Highlight the qualities, skills (i.e. leadership, vision, program, innovation, etc.), and related outcomes in which the Nominee excels as a person or organization.
Include examples of achievement in addressing the needs of the community as well as unique qualities of the Nominee.

Question 2: What major achievement(s)/innovation(s)/partnership(s) did the organization or the individual nominee's organization achieve to support this nomination?  (2,000 character limit)

Question 3: What need/problem/opportunity/circumstance led to the accomplishment(s)/innovation(s)? (750 character limit)

Question 4: Why is this organization or individual deserving of this Award? What community impacts or measurable outcomes have occurred as a result of the work of the organization or individual? Please feel free to share any other strengths and/or leadership attributes about the Nominee.  (2,000 character limit)
